Food and Drug Administration (FDA) require any pharmacy settings to update them whether there is an event of changing or adding a Pharmacist. Pharmacy setting can be drugstore, hospital, distributor, manufacturer and other establishment that is registered in FDA and being supervised by Pharmacist.  

Here are the requirements:
  1. Fully accomplished and notarized Notice of Change/Additional Pharmacist form.
  2. 2 pieces 2 x 2 Colored ID Picture
  3. Board Certificate of the new Pharmacist
  4. Valid PRC ID of the new Pharmacist
  5. Valid Professional Tax Registration (PTR)
  6. Certificate of Attendance on a FDA accredited seminar on Licensing of Drug Establishment and Outlets 
  7. Duly noted resignation letter of outgoing Pharmacist
  8. Resignation letter of incoming Pharmacist signed by previous employer
Note: Submit photocopies of items 3 to 6 but bring the original documents for verification. Also secure a copy of item 1 and have it signed by the receiving officer at FDA for your own record. For further questions, please call FDA.

  1. Good afternoon sir. I just want to ask what if gusto ng magresign ng pharmacist sa isang drugstore sino pong kelangang maglakad ng papers for change of pharmacist? yung company na po ba?

  2. Yes, company na ang in-charge dun. Make sure lang na you have a copy of your resignation letter signed by your employer. Gawa ka din ng letter to BFAD na ng resign ka na with that particular company, again signed by you and noted by your employer. Kakailanganin mo yun pag na employ ka sa ibang company na gamit lisensya mo. You will then forward a copy of that to BFAD para aware sila na hindi ka na affiliated sa previous company and you are free to work sa bago.   3. hi, good pm. kapapasa ko lang po ng board exam. so ibig sabihin po, required po ako umatend ng FDA accredited seminar on Licensing of Drug Establishment and Outlets bago ako makapagwork as community pharmacy? thank you po.

  4. Kung nag-work ka in any Drug Establishment or Outlet, Yes you need to attend FDA accredited seminar. But the company will pay for it. If you want to have it before you apply for job, that's fine. Points mo din yun sa employer, kung baga kasama na yun sa credentials mo bago ka pa i-hire ng any company at pampaganda ng resume. Yun nga lang ikaw ang mag shoulder ng expenses. Since hindi din naman ito requirement ng ibang company, ok lang din na you acquire first a job before taking FDA seminar. There's so many things to learn there and they will provide you copies of important RA and Laws.

  7. patulong naman po. kailangan ko daw po kasi gumawa ng promissory letter kasi hindi pa ako nakaka attend ng seminar. e nung tumawag po ako, wala pa daw po silang sked kung kailan ung next. mag start na po kasi ako mag work sa community. patulong naman po gumawa ng letter. thanks!

  12. The incoming pharmacist or owner can do that. Pero mas maganda na yung incoming pharmacist na ang gumawa nun. Just have everything ready, signed by the owner, and notarized.
